Conduct patient admissions and ongoing nursing assessmentsDevelop and implement individualized plans of care based on each patient's needs and goalsCollaborate closely with hospice physicians and the interdisciplinary team (IDT)Supervise and coordinate care with home health aides, social workers, and chaplainsEducate patients and caregivers about disease progression, symptom management, and available resourcesEnsure timely and accurate documentation in compliance with hospice standardsPromote a culture of excellence, empathy, and accountability in every interaction Qualifications: Current Registered Nurse (RN) License in New Jersey (required)Clinical nursing experience (hospice or home health preferred)Strong communication, organizational, and interpersonal skillsValid driver's license, reliable vehicle, and current auto insuranceAbility to travel within the assigned service area Comprehensive Benefits Package:At Vitality, we believe in taking care of our team the way we care for our patients.
