Job Description & Requirements Specialty: Case Management Discipline: Social Work Start Date: ASAP Duration: 40 weeks 35 hours per week Shift: 8 hours, days Employment Type: Local Contract Case Manager – Transitional Housing Location: Bronx & Brooklyn, NY Pay: $25–$28/hour (DOE) Schedule: Monday – Friday | Full-Time Position Type: Temp-to-Perm Position Overview: We are seeking a dedicated and compassionate Case Manager to support individuals and families in a transitional housing program . This role focuses on helping clients achieve stability and independence through comprehensive case management, advocacy, and coordination of services. Key Responsibilities: Conduct intake assessments and develop individualized service plans Provide ongoing case management and supportive counseling Assist clients with housing placement, benefits, and community resources Monitor client progress and update service plans as needed Maintain accurate and timely documentation in case management systems Advocate on behalf of clients with external providers and agencies Support clients in achieving self-sufficiency and long-term stability Participate in team meetings and collaborate with multidisciplinary staff Requirements: Bachelor's Degree required (Social Work, Psychology, Human Services, or related field) Minimum 1–2 years of case management experience , preferably in shelter or housing programs Knowledge of NYC housing systems, public benefits, and community resources preferred Strong interpersonal, organizational, and communication skills Ability to work effectively in a fast-paced, high-need environmenta25371ca-cde7-4fb6-ba87-2cf40b156c8b
