We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals, including veterans living with a severe mental illness and/or history of substance use. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 90-unit residence will provide housing for 54 individuals referred by the NYC Human Resources Administration (HRA), and the rest 36 units will be apartments for affordable to low-income households. The Case Manager is responsible for providing supportive case management services and counseling to the residents referred by HRA. Key Responsibilities:Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them Develop and facilitate therapeutic groups for residentsMaintain resident records in compliance with agency and funding standards Conduct and review assessmentsDevelop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency Monitor and ensure residents are linked to appropriate treatment and servicesProvide counseling and crisis intervention as necessaryAssist residents in securing and maintaining benefitsMaintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programsMonitor and assist residents with rent payments and budgetingConduct regular unit inspections Educate residents about approaches to mental health and substance useCompile resources on mental health information and treatment strategiesAttend ongoing training to enhance the services provided to the residentsAssist with the completion of reports as neededOther duties as assignedThe Ideal Candidate:You have a Bachelor's degree in Social Work or a relevant fieldYou have at least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settingsYou have expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniquesYou have excellent clinical and case management skill,s including knowledge of benefits in the NYC Metro areaYou are sensitive to community issues and needsYou have outstanding interpersonal and communication skills, both oral and writtenYou have strong ethical standards, confidentiality, and personal integrityYou are computer proficient (i.e., email, Microsoft Word, Excel, and PowerPoint)Bilingual preferred.
