What You'll Do:As a Clinical Specialty Pharmacist, you will be responsible for: Verifying prescriptions and overseeing patient therapy for accuracy and safety Building provider relationships and coordinating patient care Conducting clinical assessments and delivering medication counseling to patients and caregivers Monitoring patient therapy responses, documenting progress, and addressing adherence issues Collaborating with prescribers, nurses, care coordinators, and payers to optimize treatment plans Supporting individualized care plan development meeting URAC and ACHC standards Identifying medication-related issues and participating in quality improvement initiatives Serving as a clinical shift leader, guiding clinical and non-clinical staff Ensuring compliance with federal, state, and accreditation requirementsWhat You'll Bring:The ideal candidate for this role will have: Active New York State Pharmacist license in good standing Doctor of Pharmacy (Pharm.D.) degree Strong knowledge of state and federal pharmacy regulations Excellent communication skills with patients, providers, and team members Strong documentation and collaboration skillsPreferred Qualifications: Experience in specialty pharmacy or clinical care settings, especially with case management and polypharmacy Familiarity with URAC and ACHC specialty accreditation standards Outstanding problem-solving skills and the ability to work well with interdisciplinary teamsWhy Join Us in Bronx?
