Job Title: Nurse Practitioner or Physician AssistantRole Overview This is a 2-to-3-month temporary assignment dedicated to managing the health of high-risk older adults. You will address both chronic and acute conditions by combining hands-on patient care with remote management tasks, such as prescription refills and lab result reviews. Your contributions will directly impact patient outcomes through a holistic and supportive approach to geriatric care. Responsibilities Perform comprehensive physical exams and wellness assessments for older adults. Monitor and follow up on individualized care plans to ensure optimal outcomes. Develop, implement, and manage treatment plans for chronic and acute conditions. Maintain accurate and up-to-date patient records and coordinate appointments efficiently. Educate patients on preventative care, disease management, and healthy lifestyle choices. Provide both in-home and remote care, including prescription refills, lab reviews, and patient outreach. Collaborate closely with a multidisciplinary team committed to comprehensive, patient-centered care. Qualifications Graduation from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program. Current and valid board certification and state licensure. Minimum of 1 year of clinical experience in geriatrics, internal medicine, or family practice. Required Skills Excellent communication, active listening, and critical thinking skills. A compassionate, patient-focused approach to clinical care. A genuine passion for serving a geriatric population with complex medical needs. Ability to work effectively in both an onsite and remote capacity.
