Position Description: Community Mental Health Worker Positions supervised: none Hours Day/Week: 40 hours/week Salary Range: $32,000-37,000 Travel: Varies Overview: The Community Mental Health Worker (CMHW) is an integral member of the Certified Community Behavioral Health Clinic (CCBHC) team, responsible for supporting individuals with mental health and substance use needs through engagement, screening, care coordination, and linkage to essential community resources. This role focuses on identifying barriers related to Social Determinants of Health (SDOH) and ensuring individuals receive timely, culturally responsive, and person-centered support that enhances overall well-being and stability. Qualifications/Skills/Knowledge: High School Diploma or GED required; Associate's or Bachelor's degree in social services, psychology, public health, or related field preferred. Experience working with individuals with behavioral health needs, or lived experience with recovery (mental health/substance use), strongly preferred. Knowledge of local community resources, public benefits, housing programs, and social service systems. Strong communication, engagement, problem-solving, and teamwork skills. Ability to work independently in the community, manage a flexible schedule, and maintain appropriate boundaries. Valid driver's license and reliable transportation Duties/Responsibilities: The Community Mental Health Will: Client Engagement & Support Provide community-based outreach, engagement, and follow-up services to individuals with behavioral health needs. Establish trusting, collaborative relationships while maintaining trauma-informed, recovery-oriented, and human-centered care principles. Assist clients in navigating behavioral health systems, community resources, and support services. Screening & Assessment Conduct screenings for Social Determinants of Health (SDOH) using approved CCBHC tools and protocols (e.g., housing instability, food insecurity, employment needs, transportation barriers). Document findings accurately in the electronic health record (EHR) according to organizational and CCBHC standards. Care Coordination & Resource Linkage Provide warm handoffs and connect individuals to community resources such as housing programs, benefits, food assistance, healthcare, transportation, and employment supports. Track referrals, ensure follow-through, and troubleshoot barriers to accessing services. Maintain collaborative relationships with community partners, agencies, and service providers. CCBHC Model Responsibilities Support the CCBHC mission to provide whole-person, integrated care regardless of ability to pay. Participate in interdisciplinary team meetings, case consultations, and care coordination activities as needed. Assist with population health management initiatives, outreach campaigns, and quality improvement processes. Adhere to CCBHC standards for timeliness, accessibility, cultural responsiveness, and data collection. Education, Prevention, & Skill-Building Provide psychoeducation on mental health, substance use, recovery resources, crisis response, and wellness strategies. Support clients in developing skills for independent living, navigation of services, and self-advocacy. Participate in community education, events, and outreach initiatives. Documentation & Compliance Complete accurate, timely, and strengths-based documentation consistent with Medicaid, CCBHC, and agency requirements. Maintain confidentiality and adhere to HIPAA and all ethical and professional standards. Participate in required training including SDOH screening, cultural competence, motivational interviewing, and trauma-informed approaches.
