Salary USD $27.00/Hr. Description and Responsibilities Open Sky's Rapid Response Hub provides urgent and timely access to emotional, mental health and behavioral health services, all starting in one place. Individuals are coupled with a member of our team, who use the "whole person" approach to address a variety of needs including behavioral health, substance use, navigation of the medical system, housing/food insecurities and benefit enrollment. The Bridge Counseling Center serves individuals in the community as well as individuals who are served through other Open Sky programs in an outpatient capacity. We offer evidence-based practices, that include medication management and cognitive-behavioral treatments to support individuals with various clinical presentations. The LPN will support the operations of the Bridge Counseling Center and Rapid Response Hub to assure timely refills are provided and all authorizations are completed within identified time frames. They will also monitor individuals served to address and mitigate any adverse physiological or psychiatric effects from changes in medication administered while receiving services. Other Key Responsibilities: * Ensure that both programs are in compliance with medication administration policies, and all other related DPH, licensing and/or funder requirements. * Collaborate with the staff team to complete a thorough physical assessment of health needs and provide referrals to appropriate community physicians/health care providers as needed. * Provide training and consultation to programs regarding medical issues, procedures, techniques, and health & wellness practices. * Document all individual encounters and contacts made on behalf of individuals/families and development of Nursing/Health Care Plans and nursing summaries. Ensure effective implementation of Nursing/Health Care Plans through staff training, evaluation, and modification when needed. Qualifications * Valid LPN in Massachusetts, required. * Excellent assessment, documentation, and verbal communication skills required. * Valid driver's license and acceptable driving history, required. * Previous experience with adults with mental health and substance use disorders, stronly preferred.
