Our agency is State Licensed, Medicare Certified and is accredited by the Community Health Accreditation Partner (CHAP). Always Best Care is seeking a Per Diem Registered Nurse to provide intermittent skilled visits to our adult clients, offering patient-centered, one-on-one care in their residences as ordered by the physician. Responsibilities and Duties * Provide and direct the provision of home care through plan of care development and implementation of the care plan. * Communicate with physicians, other RN's, LPNs, Therapists and Other Medical Professionals. * Assist field staff in completion of the care plan and supplemental orders, and coordination of care. * Complete the documentation of skilled need and achievement of outcomes based on agency policies. * Ensure that services have been completed as ordered for timely billing. Qualifications and Skills * Graduate of an approved nursing education program. * Licensure as a Registered Nurse in the state of PA. * One (1) year experience as a professional nurse. * Experience in home health care preferred. * Ability to work autonomously and show initiative. * Demonstrated competency in OASIS documentation preferred. * Knowledge of Kinnser Wellsky EMR system preferred. * Current license in Pennsylvania as a Registered Nurse. * Current CPR Certification. * Excellent verbal and written communication, and organization skills * Must have a current Driver's license and reliable transportation.
