The RN would be a part of a small established nursing team for Pinnacle Home Care. The position entails visiting with persons served one-on-one in their private homes. Registered nurses will be assessing the needs of persons served and creating customized care plans which may include providing medication management, evaluating the conditions and needs of persons served as needed and required, and teaching the person served and/or caregivers about new diagnoses, cares, and support skills. Persons served that require home health nursing services may include persons that are terminally or chronically ill, disabled or elderly, along with persons recovering from an accident or injury. The interactions with persons served and their teams are important and can be extremely rewarding. We are looking for a passionate Registered Nurse who will be able to work daytime hours during the week, and fulfill on-call duties every other weekend. Weekend on-call schedules are flexible, and established in coordination with the nursing team. Being on-call means that you will be available to receive questions via phone. Being on-call rarely requires a non-urgent visit to a home. Preferred: RN's with knowledge and experience in skilled home health care, and/or MDH Comprehensive Home Care/DHS 245D licensing standards are preferred, however we are willing to train the right person. Experience with Medicare home care agency requirements and assessments is a plus. Schedule: Nurses will enjoy a flexible schedule. Full-time nurses are usually working during regular business hours.
