Job Description

Role Summary Create a culture of wellness by creating, promoting and implementing health & wellness education and programs for individuals and teams such as Tobacco Cessation, Cancer Screenings, Walking and Weight-loss programs and others identified by the local team as well as corporate medical services. Handle case management of FMLA, disability, and work compensation cases to include initial assessment and referral for further evaluation where appropriate. Maintains & safeguards confidentiality of employee medical notes, health records, and test results. Responsibilities * Act as the liaison between the employee / patient, supervisor, medical / rehabilitation treatment team and benefits administrator * Facilitate coordination of disability management and return to work services to employees / patients through effective management of available resources * Assure appropriateness and cost effectiveness of treatment * Facilitate decision making in establishing an evaluation program, treatment plan, and a specific return to work / disability management plan with a specific date of return to work * Serve as a liaison between employee / patient and compensation / disability management representatives * Participate in activities focused on upgrading CARE management and Return to Work services * Coordinate training for site responders including, but not limited to, First Aid, CPR / AED and Blood borne pathogens * Responsible for personal safety and the safety of those around you within the scope of the position * Participate and work with EHS team for plant and division projects * Help with all other projects and assignments needed to create a team environment Requirements Required Qualifications * Bachelor's degree from an accredited university of college * At least 1 additional year of experience providing direct occupational health, rehabilitation and/or vocational services to persons with disabilities Desired Qualifications * Bachelor's degree in occupational health, health / human services or rehabilitation is preferred * Certified Occupational Health Nurse (COHN), Certified Case Manager (CCM), Certified Registered Rehabilitation Nurse (CRRN), * Master's or Doctorate in: Nursing, Rehabilitation Counseling, Health / Human Services Administration, Rehabilitation Administration, Vocational Rehabilitation, Occupational Therapy or Physical Therapy is preferred * Working knowledge of treatment planning, vocational rehabilitation including vocational evaluation, job development, job placement, return to work, advocacy for persons with disabilities, as well as legal, financial, workers' compensation and other insurance / funding issues is preferred * Knowledge and application of basic management skills and practices * Ability to form effective working relationship with key stakeholders at the site * Strong oral and written communication skills * Strong interpersonal and leadership skills * Proficient personal computer skills (MS Word, Excel, Outlook) * We are an EEO employer for Minorities, Females, Disabled Persons & Protected Veterans. * Please view Equal Employment Opportunity Posters provided by OFCCP here.
