Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.Position Description:The Juvenile Justice Clinical Case Manager will coordinate with the Cuyahoga Juvenile Detention Center staff and Applewood Mental Health team to identify the youth in need of clinical case management to best meet the youth's needs while in the facility as well as in the community upon discharge. Clinical decisions and interventions will be driven by the needs identified in a brief behavioral assessment completed by the case manager or mental health clinician. The Clinical Case Manager will provide prevention or clinical groups to assist with skill development, anger management, managing mental, emotional and behavior symptomology as well as independent living modules. Aftercare recommendations will be communicated with guardians and court staff assigned to the youth to transition them successfully into the community. Due to youth being remanded, there is no Medicaid billing or productivity expectations and all services are provided in the facility.The following information provides an overview of the skills, qualities, and qualifications needed for this role.The Clinical Case Manager would provide, but not limited to, the following services:Complete brief behavioral assessments to clear new youth from suicide watchOYAS access to provide to clinicianCreate Special Management plans for crisis youthManage psychiatry referrals (update lists, pull folders/files, communicate with psychiatrist, get consents from parents/guardians)Provide groups on the units to meet the youth and facility needsTroubleshoot with medical about medication distribution and ordersComplete Discharge Summary and communication with guardian/POProvide support and assistance to other therapists as needed- day off coverageAttend meetings at DH (administration, medical, ODYS/ACP, population meetings)We provide individual supervision and group supervision for LPC, LSW and MFT candidates seeking independent licensure in a supportive team environment.Requirements:Bachelor level licensed social workers who have diagnostic and clinical experience will be considered.A Master's Degree in Social Work or Counseling, an LSW, LPC or MFT is preferred.Prior experience in the delivery of mental health services in the school/community to children and families with complex needs is essential.Knowledge of the Juvenile Justice system is desiredWhy Work for Us:We value our employees and their commitment to our mission and offer competitive total rewards (benefits and compensation) options to our valued employees and their families, including domestic partners.
