A healthcare network in New York City is looking to add licensed Registered Nurse (RN) to their admissions team in Brooklyn. In this role, the Hospice Nurse (RN) will be responsible for conducting assessments to admit patients to their hospice program. Assessments take place in both private homes and facilities. Admit eligible patients into the hospice program following appropriate standards of practice established by the National Hospice and Palliative Care Organization (NHPCO).Perform other duties, as neededQualifications:2+ years in med-surg, community health, oncology or hospice. Graduate of accredited School of NursingNYS Registered Nurse (RN) licenseDesired Skills:BSNPrevious hospice experiencePrevious intake or admissions experience.
