Job DescriptionJOB PURPOSE:The Nurse Case Manager, in collaboration with the multidisciplinary team and in accordance with approved protocols, accomplishes patient care by conducting comprehensive assessment of patient needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; conducts follow up with the patient to improve adherence and document in the EHR the progress and outcome during and at the end of treatment course. This includes coordinating care, assessing and identifying risk factors, negotiating care plans with the patient, monitoring and communicating outcomes.Essential Job Functions include, but are not limited to: Satisfies Core Competencies Accepts responsibility for meeting the standards of the professional, ethical, and relevant legislated requirements. Functions within the scope of practice of the governing board Clinical Case Management Conducts comprehensive assessment of patient needs and goals during the initial appointment Provides education and develops individualized plan on HIV treatment and prevention care Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation Performs clinical nursing duties including point of care testing, obtaining specimens and medication administration Coordinates care and follows up on referrals; evaluates response to treatments Provide ongoing medication adherence counseling Collaborates with medical providers, pharmacy and other disciplines to ensure achievement of patient goals and adherence to treatment plan Reporting Communicates clients' progress by documentation in the EHR; identifying treatment influences and service coordination activities to improve and enhance service continuity and effectiveness for patients. Assists in program development, outcomes and improvement by studying, evaluating, and re-designing processes; implementing changes; reviewing policies and procedures. Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.Required Education: Associate or bachelor's degree in nursingSkills, Licensure, And Knowledge Requirements Must have valid Louisiana driver's license and reliable transportation. RN or LPN license in Louisiana Current BLS certification required.Physical Demands:The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job.
