As an RN with us, you'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients. If you're ready to make a meaningful difference in healthcare, we'd love to hear from you!Note: This role requires travel within Eureka, CA and surrounding areasWhat you'll doPatient Care:Deliver nursing services aligned with individualized care plans under the direction of the patient's provider, assessing treatment effectiveness and the patient's physical and emotional responsesCollaborate in interdisciplinary planning and case conferences, ensuring care goals are relevant and feasible for the nursing team's capabilitiesRecord and communicate patient observations accurately within HIPAA-compliant systems and medical recordsFacilitate case management and care coordination to uphold patient care plans effectivelyManage assigned patients, ensuring appropriate visit frequencies and addressing evolving acuity levels and needsEducate patients and caregivers about VyncaCare services, palliative care, disease processes, symptom management, and medication managementFloat Nurse:Assist in covering patient care for other care teams during normal working hours.Your Experience & QualificationsWilling and able to provide in-person patient care up to three days a week in Eureka and surrounding areas. Travel requirement is within a 100-mile radius of Eureka.Minimum Qualifications:Valid California Registered Nurse licenseAt least 2 years of clinical nursing experience in palliative care, hospice, or home healthDemonstrated proficiency in nursing practices, clinical judgment, and documentationStrong oral and written communication skillsValid driver's license, insurance, and reliable vehicleProficiency in EMR, Google Suite, and other relevant technologiesPreferred Qualifications:Bachelor of Science in Nursing (BSN)CHPN certification or equivalentCompensation$10,000 incentive bonus. One year commitment required. $42- $52 hour: The actual hourly rate paid for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market anizational requirementsWorks with Supportive Care patients wherever they reside or are receiving care, including homes, skilled nursing facilities, hospitals and residential care for the elderly facilitiesSuccessfully performs the essential duties and responsibilities of the position through the use of physical activities as described below:Regularly requires sitting, standing, walking, talking, use of hands, listening, and observingOccasionally requires reaching, stooping, bending, kneeling, and lifting items weighing 25 pounds or lessWorks at all times within the policies and guidelines of the companyPlays an active part in the development and maintenance of good relationships with all who have business with the Company Respects at all times the confidentiality of information covering patients, staff and volunteers and is HIPAA compliant Promotes the company's philosophy of compassionate communication towards all patients, relatives, visitors and coworkersAdditional information This job description is not designed to cover or contain all job duties required of the employee.
