Job DescriptionExciting opportunity to join a team of highly motivated and energized professionals. Must be able to function at a high level with little oversight on a Monday - Friday (5 day shifts) 40 hour weekly schedule, provide nursing care, training, and home support (as needed), work collaboratively with team members, and skilled nursing home facility representatives; collect and record relevant patient health data (EMR), and develop patient care plans.QualificationsBSAAdiploma in nursing with at least 12 months of experience (3 months in hemodialysis) needed.
