Our client is currently seeking a NURSE PRACTITIONER in the Buffalo, NY area!Brief details:4 to 7 months County: Erie (Buffalo NY)40 hours per week Schedule: M-F (8a-5p)This job will have the following responsibilities:Conduct comprehensive assessments, patient facingEffectively manage acute and chronic medical and behavioral conditionsEnsure accurate and complete documentation, including ICD 10 municate and collaborate with the interdisciplinary care teamConduct advanced illness and advanced care planning conversationsProvide patients and caregivers with counseling and educationQualifications & Requirements:Required Qualifications:Active and unrestricted license in the state which you reside, or ability to obtain by start dateEducation:NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice programBoard certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:Family Nurse PractitionerAdult Nurse PractitionerGerontology Nurse PractitionerAdult-Gerontology Acute Care Nurse PractitionerIf interested, please apply to this posting directly or reach out to one of my contact methods below ASAPEMAIL: TXT: (484) 753-3515
