The primary role involves treatment planning, counseling, and working collaboratively with specialists for consultation and referral purposes.Essential Job Duties:Performs Mental Health Assessments for patients to ascertain their diagnoses and establish treatment plansProvides individual and group counseling and psychotherapyCompletes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinicsMakes andor tracks all referrals to other mental health agencies and providersWorks collaboratively with specialists for consultation and referral purposesMeets with other clinical staff to review casesProvides workshops for families on communication and other family-related issuesCoordinates emergency responses, including a team to minimize patient's danger to self and others, direct discussion with EMSFDNY regarding patient's condition, referral to psychiatric or medical emergency departments and follow-up while patient is in the emergency department or upon releaseEncourages health promotion, disease prevention, and health maintenanceParticipates in case-conference and case management activitiesSpecifics:Monday - Friday 8am-4pm (Full time only) (no weekends or holidays)Pay: $83,000 - 90,000 salaryLocation: Long Island City New York 11101 (Onsite for first 3 months, then moved to Hybrid (2 days remote, 3 days onsite))RequirementsActive LCSW License in New York2 years of experienceStrong interpersonal skillsCompassionateFamiliar with EMR documenting and reporting#Medix #MedixNE
