Position Highlights.Collaborate with the interdisciplinary team to promote and protect patientresident rights and well-being. This includes educating staff on the role of social services Participate in the development of a written, interdisciplinary plan of care for each patientresident to address social service needs Hold meetings with patientsresidents, families, and significant others as requested and to provide support and information to improve quality of life Perform patientresident discharge planning activities such as familypatient education, nursing center documentation and make referrals to appropriate agencies and suppliers Establish relationships and maintain contact and referral flow with community-based agenciesservices that benefit our patientresident population.
