Tygart seeks a full time, day shift 8am - 4:30pm Monday through Friday with weekend manager rotation Social Services Director! Position Highlights.May directly supervise two or more Social Services staff Accountable for Social Services practice, measuring outcomes to interventions, and departmental productivity Assures that a psychosocial assessment is completed for each patientresident that identifies social, emotional, and psychological needs and strengths Ensures that patientresident discharge goals are identified at admission and documented accordingly. Assesses each patientresident for discharge Educate staff regarding the role of Social Services and the psychosocial needs of patientsresidents and their familiessignificant others including the problems of aging and disability.
