Job Description:

Our client is a behavioral healthcare center with 4 North Side Chicago locations. They are seeking a fantastic candidate to work as an Assertive Community Treatment (ACT) Case Manager.Members of the ACT team coordinate and provide comprehensive recovery and rehabilitative services to mental health consumers. Interventions include a range of therapeutic and behavioral services, including crisis intervention, for individuals with both severe mental illnesses andor substance abuse disorders. Services are predominantly community based, including at consumers' homes, entitlement agencies, medical facilities, and other locations as needed. ACT services are available to consumers on a 247 basis, requiring occasional off-hour crisis response, evening and weekend coverage as needed, and participation on the ACT team's 247 on-call schedule as assigned by the team's supervisor.Responsibilities:Provide case management, outreach, and aftercare services.Assess clients functioning, strengths, needs, and diagnosis.Assess for indicators of abuse.Develop treatment plan collaboratively with client by defining goals, specific outcomes, and time frames for treatment.Provide culturally appropriate, competent and individualized linkage plans in accordance with each clients age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture.Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association of Community Psychiatrist.Develop close working relationships with DCFS and DMH funded agencies, and other mental health service providers throughout the north and west sides of Chicago and its suburbs.Qualifications:Bachelor's Degree in Social Work, Psychology, or a related field.Knowledge of recovery principles, co-occurring disordertreatment concepts, andor evidence-based treatment models.Excellent interpersonal, verbal and written communication skills.Personal vehicle, valid Illinois drivers license, insurable driving record, and vehicle insurance.
