RN Care Navigator (JACQUES Initiative)The JACQUES Initiative, the Institute of Human Virology's HIV-focused community outreach and engagement program, is seeking a Clinical Research Nurse to join our dedicated Linkage to Care team. The Clinical Research Nurse will work as part of a multidisciplinary team performing navigation services for people living with HIV (PLH) in our Linkage to Care program. The Nurse is responsible for linkage activities for the most medically complex patients our team engages while admitted to University of Maryland affiliated hospitals in Baltimore City and supports safe discharge as well as engagement and retention in HIV primary medical care. The Nurse will work in collaboration and continuous partnership with the medically complex PLH and their familycaregiver(s), clinichospitalspecialty providers and staff, as well as our full care coordination team to: promote timely access to appropriate care, reduce emergency room utilization and hospital readmissions, improve quality metrics, create and promote adherence to care plans, and increase patients' ability for self-management and shared decision making. This role is patient focused, outcome-oriented, and based on general and specialty professional standards, and functions within the multidisciplinary practice model. This role will provide the opportunity to contribute to expanding and refining navigation services, working on differentiated service delivery, supporting clinical services within community settings, and contributing to both quality improvement and implementation science endeavors. Essential Functions: * Assists patients through the healthcare system by acting as a patient advocate and navigator and promoting timely access to appropriate care. Monitors patient condition while ensuring safety and maintaining confidentiality. Identifies patient problems and makes adjustments to care plans. * Works with patients both in person and on the phone to monitor adherence to care plans, evaluate effectiveness, monitor patient progress, and facilitate change as needed. * Assists with the identification of medically complex patients using available electronic resources and databases and maintains a patient registry. * Promotes clear communication between care teams and treating clinicians by ensuring awareness of patient care plans and supporting linguistically and culturally-informed patient communication preferences. * Address social determinants of health and promote health literacy. * Track and report outcomes; conduct clinical data entry and retrieval. * Support medical evaluations, perform clinical triage, take vital signs, administer medications (oral and injectable), identify patient challenges, and adjust care plans, perform appropriate clinical documentation. * Performs other duties as assigned. Education: Bachelor's degree in Nursing, Master's degree preferred. Experience: Three (3) years of experience nursing, clinical, or in a relevant research specialization related to the position. LicensureCertification: Current Maryland license in good standing issued by the Maryland Board of Nursing to practice as a registered nurse or a multistate licensure privilege to practice as a registered nurse in Maryland or appropriate federal district licensure where research is conducted. Other: May consider a combination of directly related experience and education. Knowledge, Skills, and Abilities * Knowledge of HIV, including medical considerations and relevant psychosocial issues, and the pertinent resource landscape to address identified barriers to care; familiarity with Ryan White and CAREWare a plus. * Interest in sexual health, STI evaluation and treatment, HIV prevention (PrEP), and substance use disordersdrug user health. * Comfort with trauma-responsive principles and practices. * Comfort with harm reduction principles and practices. * Commitment to cultural humility and diversity, equity, justice, and inclusion work, including anti-racism, LGBTQIA health equity, and neurodiversity. * Excellent written and oral communication skills, including bilingual EnglishSpanish language skills.
