The Licensed Care Manager ensures the delivery of comprehensive, high-quality, patient centered care management services for Erie Family Health Centers (Erie's) high risk patients. Key responsibilities include conducting face-to-face and telephonic patient interactions, completing risk assessments, developing care plans to directly impact patient outcomes, helping patients learn to manage their conditions and providing needed psychosocial resources for a panel of 75 patients. The new team member will also work directly with providers, and other clinical staff to promote timely health care delivery, preventive care, self-management support, medication management, and discharge planning, and to address clinical or psychosocial barriers to achieving the goals of the plan of care.At Erie, we are proud to provide competitive salaries, high-quality health care plans, generous time off benefits, retirement benefits, and more! Competitive salary, annual merit increases, plus room for growth and career advancement.Main Duties & ResponsibilitiesProvides care management services and monthly follow-ups for a panel of 75 patients consistent with contractual and Erie requirements for quality and service excellence.Collaborates with patient programs, supervisor, providers, and care teams as well as uses Epic reports to identify appropriate patients for care management and identify internal support programs and services.Works with patient and patient's care team to develop, document and monitor each patients' individualized treatment care plan.Conducts chart reviews, utilizes electronic medical record, conducts brief and more comprehensive health risk assessments, motivational interviewing and medication reconciliation to identify patients' clinical concerns and acuity, risk factors, and barriers to achieving plan goals.Leverages clinical knowledge and expertise as well as Erie guidelines to support lifestyle and behavior change, offer education on clinical diagnoses and medical interventions, and provide resources to patients to support plan and address barriers.Provides care coordination to facilitate transition of care between hospital, consulting physicians, community resources, and Erie, including handling cases that require follow-up per external partner request.Documents care team communication, resulting action steps, and activities in Epic for all participating patients, aligned with Erie and CARES Act standards (e.g., no medical jargon, etc.).Other duties as required.EducationBachelor's degree required.Current Illinois licensure: RN or LPCLSW or LCPCLCSW required.Skills and KnowledgeBilingual fluency (EnglishSpanish) preferred. (May be required to assist or work with non-EnglishSpanish only speaking patients andor guests.)Strong computer skills: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).Active CPR card required.1 year experience as a licensed professional preferred.Experience working in community health or safety net settings preferred.Medical or mental health case management experience preferred.
