CARE NAVIGATOR JOB DESCRIPTION About Boston Community Pediatrics Boston Community Pediatrics (BCP) is starting a movement to change the way healthcare is delivered to all children, regardless of income. Join us! BCP is a nonprofit pediatric private practice, the first of its kind in Massachusetts. Its mission is to bring equity to pediatric healthcare by offering underserved patients direct access to their primary care provider through traditional medical office visits, telemedicine, and mobile medical services while providing comprehensive, prevention-oriented healthcare, care navigation services, and integrated behavioral health services. About the Care Navigator BCP is currently seeking a Care Navigator to join our team. The Care Navigator is responsible for providing case management services and addressing the social determinants of health (SDOH) for patients at BCP. As one of the main contacts for a caseload of patients, the Care Navigator will review the SDOH Form completed by all patients at least once a year, meet with patients individually to understand their needs and support those needs by providing immediate resources and walking them through the process. Care Navigation services include supporting housing, food, transportation and utility insecurities, childcare and education needs, and job searches, as well as connecting them to the appropriate community programs. This position also requires connecting families to outside agencies to support them with resources outside of our practice expertise, such as immigration issues, legal issues, and custody cases, among others. The care navigator may also be able to help families in creating ways through the support of BCP's hardship fund. They also will help families coordinate follow-up medical appointments as well as specialty referrals. Lastly, they will work collaboratively with the wellness navigator to connect patients with health and wellness support. The care navigator will play a supportive role, with an emphasis on patient engagement, in the coordination and execution of BCP's special events for patients which include Back to School Night, Camp Night, Job Night, Thanksgiving Meals Distribution, and BCP's Holiday Shop. Integrated Care Navigation and Wellness services are a critical component of BCP's successful model in centering patients and their needs in all that we do as we work to improve the way healthcare is delivered to all children. The Care Navigator will report to the Director of Care Navigation and Wellness Director and will be an important team member of the Care Navigation and Wellness Department.Key Responsibilities: Provide direct individual care navigation services to an assigned caseload of patients to support families with the social determinants of health, as well as navigate the complex healthcare system Assist families with housing applications, SNAP applications, RAFT applications, job applications, educational support, as well as navigating many other systems and agenciesHelp obtain healthy food resources for families, including fresh fruitsvegetables, frozen meals, grocery store gift cards Identify and develop innovative and streamlined ways to help patients overcome barriers to health, such as transportation, housing, and childcare arrangements Find creative ways to support families with the support of BCP's Hardship Fund. This is a discretionary fund that allows BCP to support patients in unique ways such as helping a family to obtain a crib or car seat, supplementing groceries at the end of a difficult month, supporting a child who needs to purchase a sports uniform, etc. Partner with established community organizations and develop new community partnerships to further support patients including USES, Casa Myrna, The BASE, One-by-One Project, Lend A Hand Society, Fresh Truck, Early Intervention, early childcare centers, Cradles to Crayons, Project Bread, DTA, Office of Food Justice, etc. Collaborate with community organizations to refer patients for further support.Gatherdevelop educational materials to support patients Collaborate with all providers at BCP Recruit patients for wellness and nutrition programs in conjunction with providers and BCP staffParticipate in various special events throughout the year in conjunction with BCP staff: camp night, job night, back to school event, Thanksgiving turkey distribution, and Holiday store including outreach and engagement with families Participate in Well Child groups, Baby Café (not yet started), support groups, youth groups, etc. to support families with their care navigation needs Track all work with patients in both an extensive spreadsheet as well as in BCP's electronic medical record, Athena Requirements Bachelor's degree in a related field (education, public health, etc.) Proficient in Spanish is required2+ years working with children Openness to and respect for other's cultures, identities, and lived experiences Organized with the ability to effectively prioritize responsibilitiestasks in a fast-paced environment Detail-oriented and able to complete work accurately and efficiently Excellent written and verbal communication skills Excellent teamwork skills Strong motivational communication skills Ability to work independently BCP offers a supportive work environment, a competitive salary, and a full benefits package.
