Job Description:

Quality Coordinator focuses efforts on outcomes that improve quality, efficiency and satisfaction of patients. The Quality Coordinator's responsibility includes day-to-day support of strategic quality and process improvement utilizing expertise in clinical support, data collection and analysis and communication of outcome results. The Quality Coordinator is responsible for leading their designated work in identifying, communicating, and facilitating best practices.Job DescriptionEducationTrainingPrevious experience with a quality role or clinical equivalentBSN preferredGraduate of an accredited nursing schoolLicensesCertification:Current Massachusetts RN licensureCertified Personal in Patient Safety (CPPS) or Certificated Professional in Healthcare Quality (CPHQ) desiredRequired Qualifications and Skills: Minimum of 2 years of experience in Quality, Safety or clinical equivalentExperience with Microsoft ExcelExperience with navigating the electronic medical recordExperience with Microsoft Power Point presentationsExcellent written and verbal communication skillsExcellent problem solving and analytical skillsAbility to analyze and critically evaluate patient care provided.Knowledge of clinical pathophysiology and related management from a multidisciplinary perspective.Preferred Qualifications and Skills: Experience with benchmark dataExperience with populating of databasesExperience with CMS and Regulatory quality measuresEssential Job Functions: Applies problem-solving and Continuous Quality Improvement techniques and tools to practice issues as outlined in the quality plan.Collaborates in determining specific quality goals and key performance indicators.Collaborates in determining the effect of quality requirements on departmental and patient focused activities.Assists in ensuring compliance with healthcare accreditation organizations such as DNV, The Joint Commission, CMS, DPH and other regulatory agency standards.Fosters interdepartmental collaboration.Collaborates in the investigation and analysis of clinical outcomes and use of outcome data.Collects and analyzes data associated with departmental quality initiatives and presents to Quality Oversight Committee.Directs and assists health care staff to carry out their patient care responsibilities by advising, facilitating, coaching, and demonstrating organized, safe, individualized patient care.Facilitates implementation of collaborative fail-safe interventions designed to prevent adverse events andor clinical misadventures.Assist leadership with risk assessments utilizing ISO standards (i.e."Swift Tools") and corresponding action plans using the PDSA model.Maintains Quality Dashboards and assigned databasesOrganizes and maintains records in a way that is predictive of trends or patterns.Collaborates with clinical leadership and frontline staff to obtain input and feedback on quality of care and patient safety.Provides ongoing evaluation and feedback of performance improvement on the basis of day-to-day outcomes, supplemented by focused reviews andor continuous quality improvement (CQI) projects as indicated.Utilizes up-to-date evidence-based standards to ensure quality standards are met within hospital's policies and procedures. Recognizes future challenges and helps to establish standardized processes that will develop the necessary capability.Collaborates with hospital leadership to actively explore system failures, to analyze and identify root causes and take action on problems that have occurred and to encourage the reporting of adverse events and near misses (great caches) both internally and externally.
