Apply Today!Job Title: Care Coordinator | Care Team AssociatePay: $18.00-$20.00hour (based upon experience)Duration: 6-months with possibility of extensionSchedule: Monday - Friday 8am-5pmLocation: ONSITE - Orlando, FLThe ideal candidate will be responsible for receiving reports, entering authorizations into the system, and coordinating follow-up tasks for discharged patients. This role will also involve responding to incoming inquiries, conducting calls for program requirements, and maintaining accurate documentation.Responsibilities:Receive reports and enter authorizations into systemWork from Health Information Exchange platform to receive patient discharge notificationsEnter tasks for follow-up after patient dischargeRespond to incoming Care Coordination inquiries from various communication venuesCoordinate and monitor documentation Care Management queuesConduct inbound and outbound calls for program requirementsPrepare daily inpatient censusQualifications:High School Diploma or GED required2+ years of administrative support experienceAdvanced knowledge of Microsoft Office productsAbility to work independently with some supervisionMedical terminology knowledge preferredExcellent organizational and customer service skillsStrict adherence to confidentiality policies and proceduresPreferred Qualifications:2+years of experience in a healthcare settingExperience working with a clinical teamPrevious experience as a receptionist or data clerkABOUT US:Russell Tobin is a leading minority-owned professional and technical recruitment and staffing advisory organization.We are comprised of specialized practices focusing on a variety of skill sets and industries.
