From entry level to management, we make artful employment placements.Salary range: $72,000-$100,000yearJob Type: Direct HireResponsibilities:Assess the psychosocial needs of patients and familiesCollaborate with internal and external teams and partners to develop care plansEducate patients and families on resources and act as patient's advocateProvide therapeutic psychosocial assessments, counseling, and crisis interventionParticipate in company events and partner programs as neededEngage on national and regional committees as requiredManage communication with external parties and organization to promote company services and programmingAssist, as needed, with resolving billing and insurance issuesQualifications:Master's degree in Social Work required3+ years of experience in a healthcare setting required Valid Licensed Clinical Social Worker in the State of Indiana preferred, but Valid Licensed Social Worker considered if currently pursing clinical licenseValid Indiana driver's license and insurance automobile requiredMust be able to occasionally travel to patient homes, schools and outreach clinicsThis role is in office until fully trained and then there will be potential to work from home 1 day per week.
