Job Description:

As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physicalcognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress.Key Responsibilities● Perform Physical Therapy assessments and develop treatment programs● Record and report patient's reaction to treatment and condition changes● Instruct patients and caregivers in therapy programs and device use● Supervise physical therapy assistant and, when appropriate, the certified nursing assistant● Prepare reports and participate in staff and Agency development activitiesSchedule and Shift Details● Monday-Friday; 8:00a9:00a-5:00p + 1 weekend per month (Saturday & Sunday; day shift wscheduled patients); 2 holidays (1 summer 1 winter); no on-callTravel● Local travel to residential homesHome Health Physical Therapist $10,000 Bonus• Compensation: PPV rates + $10k FYS bonus• Currently looking for full time, 40 hours a week; 30 points per week• Schedule: Monday - Friday; day shift (start time depends on patients); no patients past 5:00pm; 1 weekend (working Saturday & Sunday with scheduled patients) every 6 weeks + 2 holidays per year• Coverage Area: Chicago (City) and Surrounding Areas• Must have IL PT license, 1 year of PT exp.
