Compass Healthcare Consulting & Placement is conducting a search for a Nurse Practitioner to join a beautiful Nursing Facility, quick commute from Manhattan and Westchester. Qualified candidate will be a Board Certified Nurse Practitioner in the state of NY, will have some working experience in Primary Care, Internal Medicine andor Long-Term Care and will have compassion, dedication and desire to provide quality care to plan's members within the facility. Additional Pay for On-Call rotation.Position is responsible for providing primary medical care and care managementcoordination to a panel of assigned patients residing in the Skilled Nursing Facility. She will collaborate with the member's PCP and the nursing home Medical Director. She will conduct regular and acute visits assessments in accordance with the Medical Staff by-laws of the Skilled Nurse Facility (SNF) and current Federal and State Health Code. She will work with the nursing home ICT (Interdisciplinary Care Team) members.Responsibilities include, but are not limited to:Managing Residents with acute changes in condition.Advanced care planning and chronic care management.Prescribing appropriate medications, including controlled substances in accordance with NYS law.Ordering, reviewing, and following up with treatments and diagnostic municating with other nursing home staff on a regular basis and as needed to discuss changes in resident conditions.The ideal candidate will:Ideally have 2+ years of clinical experience as a Nurse PractitionerClinical background in adult, family, or geriatric specialtiesHave an ability to assess resident needs and make sound clinical decisionsHave excellent problem solving and decision-making skillsPrior experience working in a long-term care or skilled nursing facility setting (preferred, but not required)Required Qualifications:Ideal candidate must possess a graduate degree of a Nurse Practitioner program.Must have a minimum of two years full time experience as a Nurse Practitioner, preferably in clinical geriatric care.Ability to work independently, develop a differential diagnosis and initiate treatment.Board Certification from AANP or ANCC.Current DEA Certificate, petitive Salary $140,000 - $160,000, with additional generous compensation for weekend remote on-call duties & Excellent Benefits!Qualified Candidates Please Apply Now for Immediate Consideration!
