Our promise is to utilize the inherent expertise of our staff to educate consumers and serve patients and families with the highest level of dignity and grace.Job SummaryThe Registered Nurse, RN, is a member of the Interdisciplinary Team (IDT) and is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The RN initiates the appropriate interventions and support of the patient and family upon admission to Grace Healthcare Services and provides a continuous, appropriate, comprehensive and responsive plan of care. .Responsibilities and DutiesProvide skilled nursing as determined by the plan of careCoordinate the patientfamily care and services including those of the IDTEducate patientfamily and other primary caregivers in appropriate care techniquesServe as the patientfamily advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers and others. Access physical and psychosocial status of the patientfamily within 24 hours of admission to Grace Healthcare Services, update in BrightreeDevelop and update plan of care with the IDT and others involved in the patients' careMay participate in providing bereavement support to families and associated bereavement activitiesEnsure and document appropriate use of medication, durable medical equipment and suppliesParticipate in team meetingProvide accurate documentation with visit itineraries to the office. Complete documentation as outlined in hospice, state and federal guidelinesstandardsProvide appropriate coordinated care plansMonitor the recertificationreauthorization processReport necessary information to the Team Leader and other members of the IDT for on-call shift reportRespond to on-call issues as clinically appropriatePerform on-site evaluations and supervision of Grace HHAsIntegrate quality improvement results into the established plan of careParticipate in quality assessment and improvement activities as assigned by the Clinical Director andor Team Leader or QAP1 ManagerParticipate in proactive patient calls and visitsAll documentation is to be completed at point of service in Brightree and synced by end of business dayParticipate in substantive chart reviews as assigned by the Team LeaderParticipate in the orientation of new team members as assigned by the Clinical Director andor Team LeaderAttend regularly scheduled in-service and educational conferencesDevelop and achieve professional growth goals and objectives; review with supervisor on a regular basisRepresent professional nursing and Grace Healthcare Services in a positive mannerParticipate in instructional activities with Grace Healthcare Services as well as outside the Grace Healthcare Services programPromote a customer service oriented approach to care and customer relationshipsParticipate in customer conflict resolutionQualifications And SkillsNJ Registered Nurse License requiredMinimum two (2) years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experienceCurrent clinical practice and a strong customer service focusCommunication SkillsBenefits and PerksThe position includes health and dental benefits, paid time off, tuition reimbursement, and professional development.Powered by JazzHRuoFopDfVFi
