COMMUNITY HEALTH NURSEPart-time, Temporary PositionLittle Sisters of the Assumption Family Health Service, founded in 1958, is a nonprofit community-based organization that works in East Harlem to address the physical, emotional, educational, and spiritual dimensions of family health.Our Nursing Program is a Certified Home Health Agency (CHHA). We provide full-scope nursing care to first-time mothers, high-risk maternity patients, infants, pediatric patients, and the elderly. Our visiting nurses practice holistic, patient- centered care in the home setting and are patient advocates. Our partners at local hospitals and clinics rely on LSA's nurses to deliver patient care with skill, compassion, and warmth.Reports to: Chief Programs Officer andor CHHA Director of Patient ServicesThe PositionThis is a transitional opportunity to help LSA transfer our CHHA license to another provider while providing interim care once per week for 2-8 month with a minimal patient load. The expected commitment is five hours per week in person (will vary with patient load), focused primarily on maternity and pediatric populations. Specific schedule is negotiable, and some work can be done remotely. We can work around your current schedule. On call coverage and referrals may be handled remotely.Qualifications: Current license to practice as a Registered Professional Nurse in New York State; Baccalaureate degree in Nursing from an approved program. Spanish-bilingual strongly preferred but not pensation: Compensation for this role is between $50-$60 per hour, commensurate with experience and qualifications.Special Skills:Strong written, oral and interpersonal abilitiesAbility to work independently, with initiative, as a part of CHHA teamFlexibility, adaptability and reliability; accepts directionSuperior organizational skillsMajor Responsibilities:Coordinate and provide skilled nursing care to patients in alignment with the agency's mission.Apply current nursing theory, regulatory standards, organizational policies and procedures, and the physician's plan of treatment into provision of care.Develops and maintains effective working relationships with patients, families, LSAFHS staff and community agenciesResponsibilities include but are not limited to the following items:Skilled Nursing ServiceAssesses health needs of individuals and families using the nursing process (assessment, intervention, evaluation)Provides direct skilled nursing care consistent with doctor's orders and the nursing process.Coordinates services of the physical therapist, social worker, home health aide and the physician.Establishes and adheres to the Home Health Plan of Care;Demonstrates competency in carrying out Plan of Care using acceptable standards of nursing practice.Reports adverse findings promptly to the physician and nursing supervisor.Evaluates patient's progress in meeting established goals and makes recommendations for adjustment to reflect clinical status and patient preferences.Registers visit findings and submits required documentation within agency timeframes.Maintains professional skills and takes learning opportunities by attending workshops, seminars, and in-service programs; participates in research as requested by the Agency.Demonstrates knowledge of patient rights and responsibilities including HIPAA law.Develops plans for discharge and continuity of care with team members and municate with referring organizations and medical payers.Little Sisters of the Assumption Family Health Service (LSA) provides equal employment opportunities to all applicants without regard to race, color, religion, sex (including pregnancy, gender identity, and sexual orientation), parental status, national origin, age, disability, genetic information (including family medical history), political affiliation, military service in accordance with applicable federal, state and local laws
