Emerging synergistic opportunities in these spaces have positioned the company for rapid growth.Job Description: Nurse Practitioner, Primary CareThe Nurse Practitioner, specializing in primary care, will work closely with the Medical Director (MD) to provide excellent clinical services to various IntraCare Health Center locations. They will direct, implement, and provide a plan of care for patients at clinics. They will provide general preventative care, conduct check-ups, treat both acute and subacute illnesses, order lab tests, and prescribe medication for patients.Clinical direction and oversight from Physician.The clinic is located in Greenville, TX.Key ResponsibilitiesPerforms physical exams and patient observationsRecords patients' medical histories and symptomsCreating patient care plans and contributing to existing onesOrdering, administering, and analyzing diagnostic testsMonitoring and operating medical equipmentDiagnosing health issues and plan of care for patientsAdministering medicine and other treatmentsDetecting changes in patients' health and modifying treatment plans, as neededConsulting with healthcare professionals and familiesTraining patients and their families to manage and prevent illness and injuryPrescribes appropriate pharmacologic and non-pharmacologic treatment modalitiesFacilitates the patient's transition within and between health care settings, e.g. admitting, transferring, and discharging patientsFacilitates staff, patient and family decision making by providing educational toolsMonitor and provider start-up status of clinicsProvide coverage to additional clinic locations, as needed and assignedAll other duties as assignedRequired Skills And AbilitiesExcellent people management skillsApplicants must have in-office clinic experience, preferably in family medicineExcellent written and verbal communication skillsAbility to multi-task and meet deadlinesProficient computer skillsExcellent organizational and time management skillsStrong attention to detail, Strong character and integrity.Solutions focused with great problem-solving skills.Maintain high-level of professionalism and employee confidenceAbility to work independently, making sound and appropriate decisions and escalating when appropriateEducation & ExperienceA minimum of 2 plus years' experience in family medicine as NPPA Graduate of an accredited school of nursingGraduate of an accredited Nurse Practitioner ProgramCurrent Registered Nurse Licensure in State of TexasCurrent APRNNurse Practitioner Licensure in State of TexasCurrent Board Certification in adult primary care or family practice specialty areaCurrent Medicare provider enrollment with active PTAN numberCurrent TX Medicaid provider enrollment with active TPI numberCurrent provider card in Basic Life Support (BLS)Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hireCurrent DEA License, or ability to obtain DEA license upon hireExperience in geriatric care preferred Previous experience with Athena EMR, preferredStrong computer skills, including Word, Excel, and PowerPointPhysical RequirementsRequires light to medium lifting, up to 75 poundsProlonged periods of sitting at a desk and working on a computerMust be able to access and navigate each department and clinic siteMust have access to reliable transportation and ability to commute to the clinic locationPosition DetailsEmployment Type: Full-Time SalaryMonthly compensationEligible for benefitsJOB CODE: 1000078
