Now Hiring!Full-Time Geriatric Nurse Practitioner (HospicePrimaryPalliative Care)  Office Location: Signature Healthcare at Home1000 S. 16th, Suite CPayette, ID 83661 Work Schedule:Monday - Friday, 8:00AM - 5:00PM   Overview: The Nurse Practitioner will be providing acute primarypalliative care to geriatric patients in their homes; Skilled Nursing facilities, Assisted Living and Independent living facilities. Will be providing care to 8 to 10 patients per day. The Nurse Practitioner will practice independently and collaboratively to provide both acute and chronic care in facilities, residential care and home settings. The Nurse Practitioner will assess, educate, diagnose, treat and refer as indicated. The Nurse Practitioner's practice will meet evidence-based guidelines and practice service, productivity and teamwork in alignment with Signature Health care at home.   Please apply on line for this position here: caseload based on referrals from clinical staff, community and attending physicians.Manage a complex caseload of patients, rounding on patients as medically necessaryParticipate in Risk and Quality Management programs, clinical meetings and other meetings as required Perform in-depth health risk assessments on all referred patientsCollaborate with clinical team, including the attending physicians, to develop care plansReview past medical historyPerform appropriate medical screeningOrder and interpret diagnostic resultsDiagnose and treatEvaluate caseload care plans and adjust treatment as indicatedProvide prescriptionsParticipate in all mandatory compliance and other training programsSeek guidance for compliance related concerns and adheres to all applicable laws, regulations and company policies and procedures.Maintain positive relationships and rapport with coworkers, residents, family members and facility personnel.Manage time with the goal of providing 70-85% direct patient care.Support mission, values and goals of Signature Hospice, Home Health & Home CareMaster's degree in nursing required, along with commensurate experience and satisfactory completion of NP licensureCurrent RN and NP licensure in the State of practiceMinimum one year of experience as a Nurse Practitioner in geriatric, adult or family practice.CPR CertificationKnowledge of state and federal regulations with extensive knowledge of MedicareMedicaid in practicing state Demonstrated record of consistently achieving clinical performance metrics in current role.Demonstrated ability to diagnose, treat and document acute and chronic conditions.Proven ability to perform comprehensive adultgeriatric physical exam.Demonstrated ability to recognize conditions, characteristics and behaviors that place seniors at risk.Proven ability to pursue feedback, continuing education and professional development.Must be a problem solver and have exceptional oral and written communication skills.Excellent organizational and time management skills to maintain accurate records and meet deadlines.Position requires valid driver's license and adequate automobile insurance to travel to assigned facilities in regionPlease apply on line for this position here: contact: Sheena SaijoSr. Talent Acquisition SpecialistSignature Healthcare At Home E: : 503-200-7090 Signature Healthcare at Home is an equal opportunity employer committed to a diverse and inclusive workforce.
