Now Hiring!Full-Time Geriatric Nurse Practitioner (HospicePrimaryPalliative Care)  Office Location: Signature Healthcare at Home1000 S. 16th, Suite CPayette, ID 83661 Work Schedule:Monday - Friday, 8:00AM - 5:00PM   Overview: The Nurse Practitioner will be providing acute primarypalliative care to geriatric patients in their homes; Skilled Nursing facilities, Assisted Living and Independent living facilities. Will be providing care to 8 to 10 patients per day. The Nurse Practitioner will practice independently and collaboratively to provide both acute and chronic care in facilities, residential care and home settings. The Nurse Practitioner will assess, educate, diagnose, treat and refer as indicated.
