A health services organization in New York City is looking to add a new Case Manager to their team in Queens.About the Opportunity:Setting: Scattered SiteSchedule: Monday to FridayHours: 9am to 5pmPatient Volume: 22 clientsResponsibilities:The Case Manager will:Ensure that each client is receiving appropriate and effective case management servicesConduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goalsConduct face to face contacts (home visitsoffice visits) as specified in the service plansReview the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients' current needsProvide crisis intervention to clients and their families, as neededMaintain ongoing awareness of clients' medical conditions so that services are appropriate to changing needsArrange support and home health care services as necessary, making referrals to medical, home care, mental health andor substance abuse servicesProvide counseling and orientation regarding tenant's rights and responsibilities to maintain and retain housingAssist clients in locating alternative housing optionsMaintain and complete required documentation, including all contacts made with the client and any contact on their behalfParticipate in discharge planning with hospital staff for a hospitalized clientsProvide support and education to the client on budgeting to meet financial obligationsPerform other duties, as neededQualifications:1 year of fieldwork experience Bachelor's DegreeIndividual, Family and Group Counseling experienceExperience with with Mental Health illness affected populations andor Substance Use Disorder individuals.Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV AIDS, Substance Abuse, etc.Extensive knowledge of Case Management principles and practices.Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIVAIDS Program, Office of Mental Health regulationsMicrosoft office proficientSolid problem solving and time management skillsGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:Bilingual (English and Spanish)Experience in Scattered Sites ProgramsExperience using AWARDs & CARES documentation systemCustomer service and job readiness experience
