IMMEDIATE SOCIAL WORKER (MSW REQUIRED LMSW PREFERRED) CONTRACTOR OPENING!!!TOP HEALTHCARE ADULT DAY MENTAL HEALTH TREATMENT PROGRAM - NEW YORK, NYSOCIAL WORKER (CONTRACT)MSW - LICENSE (LMSW OR LCSW) PREFERREDAPPLY: PLEASE EMAIL RESUME TO #SOCIALWORKER #MSW #LMSW #MENTALHEALTHSERVICES #NETWORKESCJob Description - LMSW - Outpatient Day Program (9 - 12 month contract)As a member of the interdisciplinary care team in this outpatient day treatment community healthcare setting in upper Manhattan, the social worker triages patient care as needed focusing on same-day, short-term assistance to address issues such as:Mental health assessmentreferrals (including suicidal ideation), substance use assessmentreferral, transportation, homecare, insurance, housing, and hunger. In this role, the social worker conducts brief biopsychosocial assessments, identifies mutually agreed upon goals, and assists with developing and facilitating a plan. The social worker is responsible for educating patientscare partners about navigating healthcare services and the availability of relevant resources, coordinating with other programs, and linking patientscare partners to community organizations and services. The social worker also assesses for high risk patient eligibility for longitudinal care coordination and makes referrals as needed.Requirements:New York State LMSW or LCSW preferred.Exceptional organizational, collaborative, psychosocial assessment and intervention terest in being part of an innovative initiative to improve the quality of care by addressing barriers to primary and specialist care, preventable Emergency Department visits and hospital admissionsreadmissions.Hospital experience preferred.Proficiency in MS Office: Word, Excel, Outlook.
