Nurse, TriagePer Diem 12265 Townsend Road Nursing RNLPN DayEvening SUMMARY OF JOB: A Triage RN receives and prioritizes all incoming calls in order to continue delivery of safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provide appropriate support including education of the disease process to the patient and family. Triage team working during core business hours are facility-based positions. A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customers RECRUITMENT REQUIREMENTS: Minimum of 2-3 year MedSurg, acute care (hospital) andor post-acute care nursing experience required; 1 year of home care experience preferred; BSN preferred; ERTriage experience a plus; able to work weekend, holidays; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer). LICENSE AND REGULATORY REQUIREMENTS: Current State RN License, Current CPR Certification EOE
