Registered Nurse Case Manager-Home HealthPosition Summary: Responsible for providing nursing care in the patients' home educating the patient and families on their disease process, medications, and treatments. Providing ongoing assessment, monitoring and treatment to patients in the privacy of their homes. Essential Functions: * Demonstrates knowledge of and applies professional nursing practice and principles to patients in the home. * Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home care patient as the primary nurse. * Plans and manages assigned caseload in collaboration with Patient Care Manager; assesses schedule daily and determines priorities based on patients' levels of health care needs and payor-defined protocols. * Assures conformity to performance standards including outcomes, quality, and quantity, as defined by VNA PHILADELPHIA and other pertinent regulatory or payor entities. * Compliance with, and support for, VNA PHILADELPHIA policies and practices throughout operations and delivery of care, as aligned with VNA PHILADELPHIA Mission and Values. Other Duties: * Complete evaluation tasks, including reviewing medication and vital signs. * Assess and chart observations of the patient's condition at each visit. * Administer Physician-prescribed medication. * Identifies the need for services of other health care disciplines or agencies and initiates the appropriate referral. * Provide education to patients and families on proper home health care procedures and strategies. * Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan. * Recommend tools or devices that may improve the quality of life for the patient. * Completes records and reports necessary for patient service and administrative requirements as determined by VNA PHILADELPHIA. * Attains personal professional growth and maintains clinical competence. * Establishes and maintains effective working relationships with patients, families, peers, supervisors, and community agencies. * Meets all performance standards, including those for quality and quantity, as defined by VNA PHILADELPHIA. * Participates in interdisciplinary team conferences; facilitates coordinated and comprehensive plans and goals for VNA PHILADELPHIA patients. * Supervises Home Health Aide in completing the established plan for patient care and evaluates the effectiveness of the service. * Participates, as assigned, in implementation of the Agency's Quality Assurance Program. * Participates, as appropriate, in VNA Philadelphia educational activities, including new employee orientation and professional student clinical training and staff education programs. * Interprets VNA PHILADELPHIA services and programs to individualsfamilies and personnel of other agencies. * Participates, as assigned, in community programs. * Performs other duties as assigned. Knowledge and Experience: * Current Pennsylvania RN License. * Understanding of laws and regulations related to area of practice. * Ability to maintain confidentiality, HIPAA compliance and visit documentation. * Valid driver's license and car insurance * Ability to maintain CPR certification; must be active prior to start date. * Ability to travel %100 of time where nursing needs are present * Flexibility to work on short notice for field visits as determined by business needs. * Excellent interpersonal, verbal & written communication skills and organizational skills required. * Able to work independently and demonstrates critical thinking skills. * Knowledge of case management standards and protocols for delivery of care and other needed services. Education: Accredited RN diploma or RN degree program.
