Job Description:

Registered Nurse - Community Mental Health - Day Shift 8:30am-5:00pm!!The ACT Nurse is part of an Assertive Community Treatment team that provides mental health services directly to our members; within that team, the nurse will assess patients, administer medication, and provide education for the patient. ESSENTIAL DUTIES & RESPONSIBILITIES * Visits members at their homes or in the community to assess medical and psychosocial needs. Provides services according to nursing standards and monitors clinical status and response to treatment. * Provides consultation to the clinical team regarding members' medical issues. Serves as a resource for the clinical team on general medical issues. * Works closely with all consulting psychiatrists and medical doctors as assigned. This may include but is not limited to: medication administration, scheduling appointments, and coordinating care as indicated by the members' service plans. * Provides education about available resources or options available to the members and assist in the application process. * Advocates and assists with problem-solving to help the client access support from their community andor family. * Works cooperatively with inpatient hospital staff, keeping logs and other records of all contacts as required by agency practice. Performs and participates in crisis intervention when necessary. * May be required to be on call for emergency response coverage. EDUCATIONCERTIFICATION * Bachelor's degree from an accredited school of nursing required. * Licensed Registered Nurse required.
