The registered nurse assigned to the Primary Care - Mental Health Integration (PCMHI) team functions as part of interdisciplinary treatment teams providing care for primary care patients. PCMHI RN CM demonstrates leadership, experience and creative approaches to the assessment treatment planning and management of the complex mental health needs of these patients. The responsibilities of the PCMHI RN CM include, but are not limited to, the following: * Provides risk assessment and triage services for patients seeking access to mental health services; discusses abnormal results with referring provider and communicate normal lab results to patient or caregiver; monitors telephone calls and identify potential emergencies; coordinates patient care with other facilities and agencies; provides direct nursing care to individuals and groups of patients in accordance with the policies of the EPVAHCS. * Assesses, plans, implements, collaborates, and coordinates the needs and care of patients with acute andor chronic mental illness in an outpatient setting or in the community, using the nursing process. * Actively involved in Performance Improvement activities and committees as assigned. This nursing position will work within the Behavioral Health Laboratory (BHL) to assist in the delivery of evidence-based care for primary care patients identified with a variety of emotional and psychiatric problems. She is responsible for monitoring patients, providing education and support, influencing adherence to guidelines by providing "on-time, on target" information to primary care physicians and collaboratively making appropriate care decisions. The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR * The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript.
