POSITION SUMMARY: The Nurse Manager is a registered nurse who is responsible for coordinating services at the medical respite facility for homeless adults. This individual will provide triage of acute health problems, coordinate medical care provided by community-based healthcare providers, and function as a team member with PHMC's Health Network. All services are provided in collaboration with medical respite direct care nurses, case management staff, and behavioral health team members. The Nurse Manager will develop services and programming in collaboration with the Respite Program Administrator using National Medical Respite Standards as guidance. They will receive administrative supervision from the Program Administrator and clinical supervision from the Director of Nursing, PHMC Health Network. RESPONSIBILITIES: * Oversee clinicaloperational processes in the Medical Respite facility. * Provide initial orientation, ongoing training, and support to all respite team members. * Receive all referrals and assess appropriateness for admission to the respite program. * Provide coordination of care with medical facilities including medical emergency departments, skilled nursing facilities, and outpatient healthcare entities. * Convene and facilitate weekly team meetings to review patient progress and for discharge planning. * Work in collaboration with the social services (case managerbehavioral health worker) team to identify appropriate post-respite services for patients. * Ensure services are patient-centered, incorporating self-management plans. * Develop and promote preventive care outreach programs to educate patients on common health topics including HIVAIDS, infection control, sexually transmitted disease, hepatitis, diabetes, hypertension, obesity, and chronic disease management. * Work with registered nursing staff to ensure the provision of nursing case management including tracking of referrals, labs, and patient adherence to follow-up care. * Develop clinical protocols in collaboration with the Director of Nursing and Medical Director. * Develop and implement all medication storage and management protocols. * Manage inventory and purchasing of nursingmedical supplies. * Observe HIPAA requirements and provide patient privacy within the limitations of the site. * Provide medication management using specified guidelines outlined by the medical respite standards. * Evaluate and treat patients in accordance with clinical best practices for post-acute care in a medical respite care setting. * Provide on-call support. * Maintain positive relationships with funding sources and community organizations. * Consistently use clear, concise, and effective written and oral communication. Comply with departmental policies when sharingdocumenting relevant patient care data. * Perform other related tasks and duties as directed and assigned. * Act in the capacity of a mandated reporter to both the State of Pennsylvania and the Joint Commission. Supervisory Responsibilities: * Oversee staff scheduling to ensure 247 staffing coverage for the facility. * Develop and implement respite workflow in collaboration with the Program Administrator and Director of Nursing. * Communicate urgent or emergency needs to the Program Administrator and Director of Nursing. * Initiate the emergency disaster response plan as needed. * Coordinate and assist in the facilitation of external auditssite visits from regulatory agencies and payor sources. * Complete performance reviews for identified team members. Compliance Responsibilities: * Understand and adhere to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blower Policy, and Conflict of Interest Policy. * Keep abreast of all pertinent federal, state, and PHMC regulations, laws, and policies as they presently exist and as they change or are modified. * Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job. * Comply with the Department of Public Health (DPH), the Joint Commission, and other accreditation and regulatory agencies' standards. * Adhere to all PHMC Policies and Procedures. * Learn and adhere to the Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module. SKILLS: * Excellent verbal communication and writing skills in English * Ability to effectively collaborate with individuals who are chronically homeless * Familiarity with electronic health records (EHR) or other electronic patient databases EXPERIENCE: * Experience in primary care, community health, andor behavioral health preferred * 3+ years of nursing experience required * 2+ years of leadership or supervisory experience required EDUCATION & LICENSURE: * Bachelor's degree in nursing from an accredited program required * Must be certified as a Registered Nurse (RN) in the state of Pennsylvania SALARY GRADE: 23 EOE
