The Case Manager - Clinician is responsible for: Therapy Perform comprehensive psychosocial assessments. Provide individual and group therapy. Administer and score standardized psychological assessment measures including tests that measure intellectual and adaptive functioning and achievement. Case Management Coordinate and ensure consumer involvement in the Person Centered Planning Process (PCP). Meet with consumers face-to face as deemed medically necessary. Development & implementation of each consumers Individual Plan of Service. Participate in legal proceedings, petitioning, and testifying in clinical emergencies. Coordination of Care Facilitate the coordination of consumer's intakes, discharges, and transfers. Facilitate psychiatric hospitalizations, attend hospital meetings, and coordinate with hospital social worker. Coordinate with nursing staff to offer behavioral supports before & after appointments, accompanying consumers to appointments & obtaining relevant information when medically necessary.  Link services amongst providers and follow-up of professional recommendations. Facilitate communication with guardians, payees, and family members. Operations & Documentation Maintain consumer records. Complete and submit Service Activity Logs (SAL's). Provide consultation to direct care staff, where applicable. Participate in committees, task forces, and other related meetings and activities. About you.... You are passionate about serving adults with behavioral and mental health needs. Prior case management experience in a mental health or healthcare setting is preferred.   Education and Licensing Requirements Masters in Social Work, Psychology, or Counseling Degree You possess or are in progress to receive the one following licenses: Licensed Master Social Work (LMSW) OR Limited Licensed Master Social Work (LLMSW), Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), OR Temporary Limited Licensed Psychologist (TLLP), Limited License Counselor (LLC), or Licensed Professional Counselor (LPC).
