Plans and coordinates a plan of care for patients as part of the multidisciplinary team. This process would include the coordination of patient appointments and schedules, patient information and follow-up and delivery and oversight of patient care utilizing the nursing process and adhering to the standards of nursing practice. The nurse navigator follows the patient from one phase of treatment to another to ensure smooth coordination and good patient outcomes. The Nurse navigator develops and coordinates cardiovascular education for staff, patients and the community including support groups. The Nurse Navigator oversees data collection and reporting to meet STS, MBSAQIP and other regulatory bodies requirements. QualificationsMinimum Education: Bachelors Degree in Nursing required. Masters Degree in nursing preferred.Minimum Experience/Skills: Minimum five years of Nursing experience required. Three years of management experience required. 5 years Cardio Thoracic Vascular, 3 years Managemet experience preferred. License/Certificates/Credentials: Current: California RN license, BLS certification. Physical Demands: Prolonged, extensive or considerable standing/walking. Lift/position up to 50 lbs. Push/pull up to 50 lbs of force. Frequent reaching, stooping, bending, kneeling, crouching. Manual dexterity and mobilityAbility to move quicklyVision Requirements: Ability to adjust vision sufficiently to perform position accountabilities.Working Conditions: Regularly exposed to the risk of contagious and blood borne diseases.
