Our staff has helped DCI achieve the lowest mortality and hospitalization rates among large dialysis providers for the past 13 years in a row.The Medical Records Clerk will perform clerical duties associated with obtaining, completing, and maintaining patient medical records requests. Follows established policies and procedures to ensure effective and compliant record management. Manages process end-to-end for requests of medical records; including accuracy, minimum necessary, and submission standards. Ensures the privacy and security of all patient records. Answers and facilitiates requests for medical and billing records from internal and external parties.Responsible for retrieving/collecting medical records and information from the EMR system.Review accuracy of records prior to release to external parties.Handle record invoicing per state and federal laws.Maintain accurate logs and information release forms for medical record requests.Research lost or missing records/information in accordance with established procedures.Set up reviews for external chart reviews, provide training, and address questions.Distribute records and reports to departments as requested.
