Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.HHC Payroll Title: Social Worker Level IIFunctional/Office Title: AOT CoordinatorSummary of position:A significant function of the Social Work reentry department includes identifying individuals eligible for Assisted Outpatient Treatment (AOT), submitting appropriate referrals, coordinating with the DOHMH/Rikers AOT team and coordinating with community providers. This full-time position reports to the Director of Social Work through the Clinical Coordinator of Social Work and is responsible for providing coordination between CHS and the DOHMH's AOT Rikers team, with the purpose of ensuring that individuals with Serious Mental Illness who are eligible for AOT leave the jail under an active order and with the appropriate follow up and supervision.Responsibilities Include:Providing education and supervision to other social worker staff about the eligibility and overall process of AOT. Screening AOT referrals for completeness and appropriateness.Tracking status of pending AOT referrals and helping assure completion of required documents/information for DOHMH/AOT review.Facilitating communication between CHS' treatment team and DOHMH/AOT.Facilitating communication between DOHMH/AOT and criminal justice system stakeholders (e.g., attorneys, parole, court) and community treatment providers, including housing providers and alternative to incarceration programs. Cross-referencing data systems for status updates and corrections on AOT referrals.Tracking and communicating status changes to those referred to AOT (e.g., transfers to state hospital or state prison).Coordinating care for those already under AOT who become incarcerated. Organizing, participating, and/or leading regular administrative and clinical meetings with DOHMH/AOT. Department Preferences:Experience submitting and coordinating AOT referrals.Clinical experience working with individuals with mental illness.Clinical experience working in with individuals involved in the criminal justice system.Experience facilitating communication between stakeholders, including court personnel.Excellent organizational skills.Prior administrative organizational experience.Strong data tracking skills.Qualification Requirements:Master's degree in Social Work with active NY State license in good standing.LCSW preferredExperience -At least two years of post-license experience.An equivalent and satisfactory combination of education and experience may be consideredFor Assignment to Level II1. Valid license as an LMSW issued by the NYSED; or2.
