Job ***** Case Manager (Contract to Hire)***** LHH Recruitment Solutions***** Chatsworth (Main Office) Chatsworth, California 91311***** $25/hourWork ***** Monday - Friday, 8:30am - 5:30pm (Onsite)Client ***** Bachelor's Degree in related field (highly preferred, but not required with experience) Minimum 1 year of experience in Social Work or Case Management Valid Driver's License and Insurance Completion of first two (2) COVID-19 shots + one (1) booster required Job *****Participate in the development of program plans for individuals, monitoring, evaluating, and revising them as necessary.Identify and coordinate services for individuals and their families.Document progress, complete forms, and reports in accordance with regulations and NLACRC policies and procedures.Advocate for individuals served by community agencies.Schedule and attend interdisciplinary meetings with individuals served, family members, service providers, advocates, etc.Identify barriers to service delivery and implement culturally sensitive strategies with empathy, trust, and fortable working in underserved and non-traditional office settings to meet community needs.Rotate officer-of-the-day duties, assist co-workers with special projects, provide case-coverage as needed, and provide emergency on-call services when required.If you possess the experience and meet the requirements outlined above, we welcome you to apply and join our team at LHH.
