LHH Recruitment Solutions is searching for a full-time Case Manager on a contract basis for a growing client in Lancaster, CA. Our client is seeking individuals who are highly dynamic and adaptable, empathetic, and committed to addressing the unique challenges faced by their clientele. This is an onsite position, and the candidate needs to leave in or near Lancaster, CA. ***** $ 25/hr ($26 for bilingual) Job ***** Contract to hire ***** Monday - Friday 8:30am - 5:30pm ***** Conduct thorough and empathetic assessments to identify the specific needs, strengths, and obstacles of individuals or families seeking terviewing clients and conducting a detailed review of their history to gain a comprehensive understanding of their situation and long-term objectives.Collaborating with clients to design individualized case plans that outline both short-term and long-term goals, tailored to their unique circumstances.Identifying suitable resources and services to meet the clients requirements and facilitating connections with relevant support networks.Coordinating efforts with other service providers and community organizations to ensure clients receive comprehensive and timely support.Serving as a dedicated advocate for clients, safeguarding their rights, and providing guidance to navigate complex systems successfully.Offering immediate assistance and support during crisis situations, ensuring clients receive the urgent help they need.Providing guidance and training to enhance clients independent living skills, empowering them to lead fulfilling lives.Regularly check in with clients to monitor their progress, reassess ongoing needs, and make necessary adjustments to the case plan for continuous improvement. ***** High school diploma/GED, required2+ years of experience in a related field, required.Bachelors Degree in a related field, required or equivalent experience would be considered.Covid vaccination, required.Spanish speaking, preferred.Reliable transportation, required.Valid driver's license & Car Insurance, required.Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
