Description AHRC New York City, a family governed organization, is dedicated to enhancing the lives of individuals with intellectual and developmental disabilities and their families. Under the direction of the Director, the Community Support Supervisor facilitates Person Centered Planning and coordinates individualized services. Supervises the Community Support Professionals(CSP), ensuring effective service provision. Promotes independence and community inclusion of individuals served through creative planning, effective use of staff management and professional teamwork. Develops individualized plans with CSP and ensures compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies. General Responsibilities: + Develops direction for service with clear philosophy and expectations toward personal future planning and community inclusion; develops activities to implement this philosophy. + Through assigned staff, fosters independence, productivity and community inclusion of the individuals served while protecting their health, safety, well-being and civil rights. + Ensures that individualized habilitation plans are consistent with individuals’ service plans and are supported by the team; follows through on each individual’s established outcomes and documentation of progress by the Community Support Professionals. + Facilitates and supports the development and maintenance of individual support networks through outreach to family members, community members, and professionals and through coordination of personal planning sessions in collaboration with the individual served. + Assists in hiring and training process for Community Support Professionals; supervises staff on an ongoing basis either individually or in small groups. + Coordinates and participates in direct support staff training and inservices by orienting new staff, arranging for staff to attend training, maintaining training records, and supporting ongoing staff development. + Evaluates assigned support staff on an ongoing basis implementing a job performance evaluation annually; develops appropriate goals for staff and follow through on staff progress. + Assigns staff various duties particularly in the case of staff absences; ensures that there is adequate supervision for the service needs of the individuals served at the program. + Facilitates effective communication with employees and outside agencies for ongoing development of community opportunities. + Functions as liaison between Service Coordination Services and Day Habilitation service. + Participates in any special case conference, staff and parent meetings as appropriate. + Ensures that individuals supported are safe and living healthy lives by monitoring safety issues, coordinating, monitoring and documenting medical supports, practicing appropriate emergency procedures, responding to emergencies and promoting individual rights regarding health and safety issues. + Enhances staff relations by using effective communication skills, encouraging growth and self-development, facilitating teamwork, employing conflict resolution skills, and providing adequate supports to staff. + Ensures that program remains in compliance with New York State Home and Community-Based Waiver Services regulations, agency philosophies, and policies. + Other duties as assigned. Qualifications + Bachelor’s Degree in the field of education, social work, psychology, vocational rehabilitation, guidance and counseling or other related human services field from an accredited program, with at least two years of experience in the field of disabilities. + Knowledge of Home and Community Based waiver programs -- and ability to keep up to date to such knowledge – required. + Knowledge and understanding of Person Centered Planning. + Ability to establish and maintain a working relationship with program participants, families, staff and community agencies. + Ability to supervise and provide role modeling to staff effectively. + Strong verbal and written communication skills necessary. + Ability to organize work with self and others, as well as delegate work to others as appropriate.
