Job DescriptionPosition Title: Licensed Practical Nurse (LPN)FLSA STATUS: Non-ExemptREPORTS TO: Nurse ManagerSUPERVISES: NADEPARTMENT: Home HealthCOMPANY: Overlook Home Health ServicesPOSITION SUMMARY:The LPN provides individualized nursing care and comprehensive services to patients and families in their own homes under the supervision of a Registered Nurse.PRINCIPLE DUTIESESSENTIAL FUNCTIONS:The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.Provides basic patient care under the supervision of a Registered Nurse in accordance with physician's ordersObserves patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary actionEducates patient and families in self-care and health managementAttends case conference as requestedAdheres to policies and proceduresParticipates in agency quality management and safety activitiesQUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
