Join Our Growing Team!The Nurse Practitioner is responsible for in-home medical office-type visits. The Nurse Practitioner is paired with a Medical Assistant to perform in-home visits in our company vehicle. Visits include review of patient history, physical exam, assessment, diagnosis, and plan including prescribing medications, ordering ancillary services, etc. All findings are documented in our EHR and shared with other members of the care team. Currently looking for Part-Time availability any 2-3 weekdays per week (8hr Shifts) with potential to lead to full time. Responsibilities include, but are not limited to:Clinical history-taking and medical chart reviewExcellent physical assessment skillsSolid comprehension of pharmacology and prescribingAccurate diagnosing and plan developmentOrdering any medications, DME, or ancillary services required by our clientsCommunication with members of the care teamRequirements:Must be 21 years of ageMaster's Degree or higher requiredAt least 1 year experience as a Nurse Practitioner and at least 2 years experience as a Registered NurseActive and unencumbered license to practice as an APRN in GeorgiaActive certification by the ANCC or AANP Valid Driver's License clean driving recordMust pass a drug screen and background checkJob Type: Part-timeBenefits for Full-Time:401(k)401(k) matchingDental insuranceHealth insuranceLife insurancePaid time offVision insuranceSchedule:Varying weekdays 9am-5pm(MWF) 8 hour shifts per day preferred or TuesThur with potential for full-timeWork Location: Station-based using mobile care unitsPay:$35 hr + $35 per patient seen1 per hourAdditional $10 for each Transition Care Management Patient seen
