Summary Job DescriptionLG Health Physicians Family Medicine Lincoln is seeking a Nurse Practitioner or Physician Assistant to join their team. As part of a highly productive team, consisting of 5 Physicians and 3 Advanced Practice Providers, delivery of comprehensive, quality care for patients of all ages - from newborns to geriatrics is our specialty. The provider team emphasizes wellness and prevention, helps patients manage chronic conditions, provides timely care in the event of an illness or injury, and brings direct access to the most advanced treatment options.Family Medicine Lincoln has two offices that serve our patient population. We share staff between the two offices and function as one practice.Family Medicine Lincoln continues to be recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). A Patient-Centered Medical Home is a model of care that strengthens the physician-patient relationship through accessibility, excellent communication, and utilizing the latest information technology. Our team based care approach ensures patients receive the highest standard of care with direct access to services to promptly meet each patient's needs.Hours: Full time with hours primarily between 7:00am-8:00pm Monday-Thursday, Friday 8:00am-5:00pm, and a rotational Saturday from 8:00am-12:00pm.
