Job Description:

Tanner Health System is looking for a Nurse Practitioner who will become the Coordinator of the Structural Heart (SH) Program. This provider will plan, develop, implement, and manage the Structural Heart Program under the direction of the Heart and Vascular Executive Director and Cardiology Medical Director. Structural Heart procedures include, but not limited to, MitraClip, TAVR/ TAVI, LAAO, PFO/ASD, paravalvular leak repair and other complex structural heart procedures.Must demonstrate strong leadership skills, exceptional communication skills and the ability to collaborate with physicians and other members of the multidisciplinary health care team. Collaborates closely with the Cardiac Surgeons, Cardiologists, Interventional Cardiologists and other members of the health care team in providing care for structural heart patients. Manages and coordinates all aspects of the SH program to facilitate seamless care from referral to follow-up for patients who are candidates for SH therapy.RESPONSIBILITIES:Facilitates a team approach and collaborates with the multidisciplinary SH team to establish a plan of care starting from initial interventional evaluation and following the continuum of care including pre-procedure, post-procedure, and continued follow-up post SH implant.Provides medical care and treatment within established scope of nursing practice to hospital inpatients and valve clinic patients as assigned by and under the direction of the collaborating physician.Plans and coordinates health care education for patient, family and other health care providers as needed.Assist in leading the facility to accreditation compliance and regulatory alignment, improvement on national and state/local public-reported measures, focused clinical reporting, and compliance with national programs.Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies.Participates in quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise.Specific responsibilities will include scheduling outpatient evaluations and follow-up, implantation scheduling, maintaining valve database, and overseeing documentation (creation of order set, etc.) of the patient experience.QUALIFICATIONS:Education:Graduate from school of nursing.Masters degree required, NP preferred.Experience:Minimum of 5 years' experience with cardiac/vascular surgery and/or invasive cardiology.Skills in physical assessment, lab data interpretation, technological skills, and nursing theory and practice are required.Teaching experience in patient/family and staff education required.Certification:ACLS/BLS required.Certification in Critical Care with Cardiac Surgery sub-specialty or other related certification preferred.License:Current license to practice as a Registered Nurse Practitioner in the State of Georgia is required.
